Many people living with HIV are now ageing, and a higher percentage experience multiple health problems.
Bolton Clarke has a specific HIV nursing team that can provide support for you to stay at home longer.
Our services include:
- – Mental Health support
- – Chronic pain management
- – Medication Management
- – Wound Care
- – Continence assessment
- – Palliative Care
Our specially trained HIV nurses will come to you in the comfort of your home, and can engage the services of our wider nursing team to do daily visits if required.
This is a free service, open to long-term survivors, families, refugees, asylum seekers and people newly diagnosed with HIV.
No referral is needed to access this program and you can refer yourself to the service.
